Here are the instructions on how to use this Gift Certificate:

1.  Print out the Certificate using the print icon at the bottom of the image below.

2.  Call 1-866-985-5262 and get a serial number specific to you for the amount of the Gift Certificate that you want.

3.  You may pay with either Visa or MasterCard.

4.  Write the serial number, name of recipient, and amount of certificate on the Gift Certificate that you printed.

5.  Remember to bring the Certificate with you when you come to your appointment!
